Our new patients, please prepare for you visit and bring the following items with you to your appointment.
Currently we take over 100 insurance plans. Insurance status will be verified before your appointment , so please notify the office of any changes. All office fees are payable at the time of service. We will file your insurance claim for you, but patients are responsible for paying all co-payments, deductibles, and for all non-covered services and fees.
Appointments are scheduled from 9:00 a.m. to 4 p.m., Monday - Thursday and 9:00 am to 12:00 p.m. Friday. To avoid a cancellation fee please notify us 24 hours in advance of cancelling your appointment.
Due to the nature of our practice, occasionally, the doctor's schedule and hospital emergencies may necessitate a change in your appointment. If this occurs, we will do our best to contact you to avoid an extended wait.
After business hours phone calls are forwarded to our answering service. The answering service will contact the covering physician. If you are having an emergency, please call 911 or go to the nearest hospital emergency room.
When you need a prescription or refill, please plan ahead and order any prescription refills during business hours (Monday - Thursday). We will not be able to refill prescriptions over the weekend. To expedite refills, have available the medication, dosage, and pharmacy telephone number.
